A derivative of my v3 IC-only module. Same size/cost as v3. Untested, will update.
The difference? This board is self-contained, and adds pads for a resettable fuse next to the USB header pads. (No, not specifically pads for a type-A USB male plug - you supply a cable and plug like this. Uses standard USB order of +5v, Data(–), Data(+), GND.)
However, because of its double-sided nature (rather than riding on a single-sided carrier), it’s not very well suited for the slide-in bay on my Modul-Ar-duino.

For using differently-wired tapes from different sources together; panel of 4 ports, break-apart (if you want, or you can wire more together to use them as distribution boards).
Use ONLY ONE jumper/wire per 2x4-pin jumper block to avoid short circuits - place jumper or wire vertical in each block when viewing the board oriented like the board thumbnails.

Odds are high that it won’t stay at the 5000th position for long due to churn, deleted boards, etc. But it’s nice while it lasts. Will add screenshots [later] to prove if it indeed got the #5000th position (at the time). #ToEdit
What a neat milestone for OSH Park!
$10.60 / set of 3. Untested, but fully expected to work, and I decided to go ahead and release the next (smaller & simpler) version first, as it’s probably more versatile. I just sat on this design til now.
Works for either SMD or thru-hole DIP packages of amplifier IC.
(note SMD=0.4W & DIP=1.4W Pd-max)
Uses 3x2 header for volume pot,
5 large electrolytics=470uF 10+v,
2 small input ‘lytics = 4u7 25+v,
input load R6 & neighbor= 10k (x2),
input bias R4 & R5= 3k3 (x2),
all 4 ceramic/film caps =0u1 25+v,
output snubber R (next to C10, C11)= 4R7 x2,
power led & R (between snubber-R’s)= 4k7 [both optional, if you want to save power.]

$7.55 per set…for my Modul-Ar-duino, uses FT232RL and either USB mini-A or Std.-B connectors
Has 3v/5v IO jumper, DTR/RTS jumper and cap after it for arduino auto-reset (optional shorting jumper too).
Edit: Bug in Ground plane polygon fills - isolated by Eaglecad from the vias. Was learning how to use polygon tool at the time. Noob symptom, sorry.
